ACL's Commit to Connect initiative is hosting a webinar on how falls prevention programming can be a powerful intervention for maintaining and rebuilding social networks. Speakers will share the role of falls prevention programs in addressing social isolation and loneliness and how they are scaling their impact.

Today marks the start of Older Americans Month (OAM) – a time to recognize older Americans’ contributions, highlight aging trends, and reaffirm our commitment to supporting older adults in our communities. This year’s theme, Champion Your Health, emphasizes prevention, wellness, and personal responsibility as foundations of healthy aging. It encourages people to take an active role in managing their health, advocate for themselves, access preventive care, and make informed decisions that support independence.

The Health at Home Challenge will recognize and reward partnerships between health plans, health systems, and community-based organizations that support whole-person health for dually eligible and near-dually eligible Medicare and Medicaid beneficiaries.

Participants will learn how to recognize early signs of cognitive impairment in shelters and temporary housing settings and gain practical strategies for engaging seniors in ways that foster meaningful participation.

Hosted by ACL’s No Wrong Door Resource Center, this webinar will highlight two new resources that define the essential knowledge, skills, and abilities frontline community-based service providers need to effectively deliver person-centered support. The session will highlight Southern Alabama Regional Council on Aging’s Community Care Solutions and the promising practices this CCH uses to train the community workforce.

This competition is designed to improve health, independence, and affordability of care for dually eligible Medicare and Medicaid beneficiaries by scaling advanced community care networks, led by community care hubs that integrate health care and community care.
